anyone gets a lerner's permit recently?

I checked the DMV website, and there is one requirement which is

" Pass an eye test. You may also have en eye care practitioner complete Form
MV-619 (Eye Test Report).

anyone knows where I can get the test?

can I just take it in DMV office?

or I need to find a health care professional to help me complete the form

before I go there?

If so, how much and where shoud I pay and go?

btw, if I have SSN card, I just need to bring it, my passport and ATM card

there directly without going to SSN office to fill out some forms right?
